Added stable per-output diff data for machine consumers.

  • --json now exposes a changes array with one entry per generated output.
  • Each updated output carries its own unified patch.
  • Human-readable diff output and JSON now share the same underlying change model.
  • check --json exposes repository-only changes without inspecting global output files.
