Add a vertical layout for sqlcmd results

Add a switch or variable for printing results vertically. Each line would have the column name followed by the value.

if there are multiple 'rows' of results, should those be converted into columns, or is each row still printed sequentially but the format is columns are also printed vertically - hence, the column names would be repeated with each row?

I believe it's the latter - if there are N columns the column name repeats every N rows of output.
