Fixes

• Require a completed, error-free response and an idle transition before automatic queue replay.
• Keep queued work paused after dismissed questions, rejected permissions, aborted runs, and errors.
• Recheck interruptions before submission and throughout carry operations, preserving entries that have not been sent.
• Ignore late cleanup from earlier runs and allow deferred entries to resume after the current run finishes successfully.

Explicit /queue:start and /queue:flush remain available to resume queued work.

OpenCode API limitation: cancellation after the final assistant response is marked complete can still be indistinguishable from natural completion.
