Documentation

No change in behaviour. The README is shorter and links to guides in docs/. The setup guide
and the architecture document are gone:

• Troubleshooting:
  every symptom, its cause and the fix.
• Single-worker hosts and upgrades.
• Coming from h5p-standalone:
  its options by their names here.

The README's no-build-step snippet now pins the version of the element and the runtime on the
CDN. Copy h5p-sw.js from the same version, and copy it again whenever you change it.

Corrections:

• To reload the same package, remove src and set it again; setting the same URL does nothing.
  This is also how to restart content after clearUserData().
• An error event while state stays ready can be a media file that failed to extract
  (network, quota), not only a runtime exception. Hide the player only when state is
  error.
• progress.fraction is null while the total is unknown.
• Under resume="host", the userData you hand back is not checked against the build; compare
  the revision you stored with it.

A player statement that carries context.platform but no context.revision is now relayed
instead of dropped. The element sends such a statement on a host without Range when the page
is hidden before the package's index has arrived, so as not to lose the learner's record.
