Content written for an older library version is upgraded before it plays

@missing-elements/h5p-runtime 0.1.1 and @missing-elements/h5p-offline-player 0.6.1.

When libraries come from a bundle, such as libraries="pack" or your own copy, the player serves
the newest minor version it has of each library, including to content written for an older one.
H5P libraries ship a step in their upgrades.js for each minor version that changed the shape of
their parameters, and the H5P plugins run those steps when a site installs a newer version. The
player now runs them too, in the browser, before the content starts.

A Question Set written for 1.20, for example, lost its background image when played against the
pack's 1.21, which reads the image from a new place. It now keeps it. Of the pack's 47 libraries
with upgrade steps, 21 have one for their current version, among them Question Set, Interactive
Video, Summary, Drag and Drop, Image Hotspots and Dialog Cards.

If a step fails, the content plays as written, as before, and the console says so.

Also fixed: statements from a content type with a 0 in its version (Accordion 1.0, Crossword 0.5:
15 content types in the pack) carried a broken category, such as
http://h5p.org/libraries/H5P.Accordion-1.undefined. They now carry H5P.Accordion-1.0.

The runtime does the upgrade, and the player's worker tells it which libraries have upgrade steps,
so it asks for nothing that is not there. Each works with the other's previous version: with
player 0.6.0, the runtime asks for every library's steps and takes a 404 as none.

On a CDN, update both URLs: h5p-offline-player@0.6.1 and h5p-runtime@0.1.1.

@missing-elements/h5p-embed 0.3.1

No changes of its own: it requires the new player and runtime. Write and deploy your player
domain again to get the upgrade there.

@missing-elements/h5p-verify 0.1.3

No changes of its own: it requires the new player and runtime, so --libraries pack upgrades
content the way the player does, and a failed upgrade appears in the report as a warning.
