Back no longer empties the player in Safari

@missing-elements/h5p-offline-player 0.9.3.

In Safari, on a page with the element at the top level, the first Back after a package opened sent the player's frame back to a blank page. The content disappeared and the page stayed where it was. Opening a package had added an extra step to the page's history. Chrome and Firefox add none.

Now opening a package adds no history step in any browser, so Back leaves the page, or goes to the host's own previous step, as it does in Chrome and Firefox. Switching between packages and clearing the player added no steps before this release either.

Only Safari (macOS, iPhone and iPad) was affected, and only when the element is in the top-level page. An element inside a frame, such as /embed on another site, was not.

Documentation: Interactive Book and Game Map report a part's result as their own

The README's finished row now says that Interactive Book and Game Map send a chapter's or a stage's result as the content's own, as they do on any H5P site. The content's own result comes last; for Interactive Book, only with reporting. A host that takes the last finished gets the content's result. cmi5, which records the first, completes such a unit on its first part (see docs/cmi5.md).

The runtime stays at 0.2.2. On a CDN, update the player URL: h5p-offline-player@0.9.3. Embed 0.6.1 accepts 0.9.3, so a player domain gets the fix when its site is written again with 0.9.3 installed.
