Add new Text box

I am new to Query Builer and was wondering how can I add a new field(textbox) as shown in the below picture to the default form using stand alone query builder.

I guess you want to get the value of this input, not just display it...

Create a new operator (http://querybuilder.js.org/#operators) with nb_inputs: 2 and enable it on your filter (read this http://querybuilder.js.org/#filters)

@mistic100 - you are right, I want to add the text box and get the value out of that control at runtime.
I was able to achieve below following your instructions

 operators: [
                    { type: 'equal', nb_inputs: 2},
                    { type: 'not_equal', nb_inputs: 2 }                     
                ]

and provided that to my querybuilder options for rendering

Is that what you were suggesting? Also , how can I add that custom text('then') before the textbox instead of comma(,)

what can be done if i want one input field as text and other as dropdown??
