Fixes

• Disable background tasks and deferred work in headless sessions (claude -p). Previously, these sessions could start long-running tasks but would then end and kill them, causing repeated retry loops with no progress. Long commands now run in the foreground or as detached, polled tasks.
• Fix --release-notes flag being ignored when changelog.disable: true in GoReleaser configuration.

• Add intro video to README showcasing issue labeling, confidence gating, product-owner design, UAT and code review, fleet management, and cost dashboards.
• Expand README documentation with comprehensive coverage of structured-output pipelines, concurrent execution slots, confidence gates, entry outcomes, spec → plan → execute chains, completion gates, post-ship code review, and ai-resolve-merge workflows.
• Add docs/how-it-works.md guide.
