Minor release on top of v1.9.2: Doctrine DBAL 4 support without Doctrine ORM.

• Require doctrine/dbal ^3.10 || ^4.5 directly instead of doctrine/orm ^2.19.5. The package only used DBAL, and ORM 2 pinned DBAL 3, which kept symfony/http-foundation on 7.4 for consumers and pulled the abandoned doctrine/cache.
• Use the API shared by DBAL 3.10 and 4: fetchAssociative()/fetchAllAssociative()/fetchOne(), executeStatement(), createSchemaManager() and bindValue() with ParameterType. The Loader relation deletions now report the affected rows.
• Debug connections log through a DBAL middleware with the new Utils\EchoSqlLogger (DBAL 4 removed EchoSQLLogger): each statement is echoed as before; the connection message, which carries the credentials, is skipped. psr/log is now a direct requirement.
• Standalone commands pass '' instead of null to getopt() (deprecated since PHP 8.1).

The minimum PHP version becomes 7.4 (DBAL 3.10); older platforms keep resolving 1.9.x.

Verified on PHP 8.5.12 with DBAL 3.10.6 and with DBAL 4.5.0, inside a Laravel 13 application against a disposable MySQL 8.0: 41 smoke checks with no warning logged and 20 direct checks of every changed call site (fetch helpers, debug logger, getUrlData, Utils\Editora, Loader bulk import and deletions, AppModel, Generator schema manager, Clear). Lint clean on PHP 8.5, 8.3 and 7.4.
