Fixed:
- Streamed fx.run resumes a parked tool approval instead of replaying the run id
- A failed agent-event append retries once, then ends the stream
- Memory SQL accepts CREATE INDEX IF NOT EXISTS so identity tables can be ensured
- create-oke's Dragonfly stub escapes the maxmemory default so the scaffold parses
- journal:runs writes do not open a store, and unknown client facts stay off the request card
