v0.24.1 — 2026-10-10

♻️ Changed

  • MCP confirm stays pending when the confirmation is rejected. See Security.

🐛 Fixed

  • The release workflow grants ci.yml its contents: read, issues: read, and pull-requests: write permissions. A smaller grant fails at startup, before npm, JSR, or the GitHub Release run.
  • Postgres exec reads count and affectedRows, so a leased durable flow no longer throws OKE1074 on Bun 1.4.2.
  • 0.23.2 and 0.24.0 do not work with Postgres on Bun 1.4.2. Upgrade to 0.24.1.
  • Cache invalidation is skipped only when a fresh read of the instance registry shows no other live instance. A snapshot that says the process is alone is read again before the skip, so a peer that registered during the last poll is still notified. A missing table, a registry that has not been read, or a snapshot older than one poll still publishes.
  • A channel receipt update on an instance that has not reloaded loads the stored row, including a provider message id that exists only in the body, and writes the new status before it returns.
  • Two instances installing the oke.* row-security helpers, or creating the same table on the first insert, no longer fail the request with tuple concurrently updated. A clock handler that is killed mid-run is tried again when its lease expires.
  • The Redis competing-consumer test reads the process-start REDIS_URL, so a suite that clears the variable cannot skip it.

🔒 Security

  • MCP confirm uses confirmationId. Confirming is a different session. A rejected confirm does not consume the pending confirmation or the token.
