Added support for bash compeletion of
type=click.Choice Options and Arguments.

Added comprehensive tests for bash completion.
There are many scenarios present for arguments.

It looks like this doesn't work for the case of --opt=.

I'll get a fix up for this tomorrow.

Improve completion by delegating to Click
Previously we generated our own completions for available options and
commands, in a similar way to how this was is done in the
`click._bashcomplete` module.

However, this module has had various improvements made to it since this
was originally done. Rather than duplicate similar code here (and have
to do this again in future), this commit changes our completion to
delegate generating the available completions to Click. There is also
additional handling around this to still display relevant help alongside
the completions, where possible.

The end result is that this commit should make click-repl's completion
function as before, except with some additional completions shown where
these are relevant, such as the completion of choices added in
pallets/click#681. Any additional improvements
to Click's completion will also become available to us.

Handle completion of Choices in Tuple Options
Completion of Choice option values was recently added, in
pallets#681. However, this does not cover
Choice values within Tuple options. This commit adds similar handling so
completion will cover these option values as well.

Explain plans were not generated when using psycopg2 named cursors

When using named cursors in psycopg2, the agent attempted to generate an explain plan using the same named cursor. This resulted in a syntax error when the query was issued to the database. When using the default connection and cursor factories, the agent will now execute the explain query using only unnamed cursors.
Convert bytes-like SQL statements to strings before obfuscating

If a bytes-like object is used instead of a string when making a SQL call, a traceback was seen in the logs with TypeError: cannot use a string pattern on a bytes-like object. This issue has now been fixed.
Save settings to MessageTrace objects

If an external call using an instrumented http external library (for example requests) was used within a MessageTrace, a traceback was seen in the logs with AttributeError: &#39;MessageTrace&#39; object has no attribute &#39;settings&#39;. This issue has now been fixed.
