github-wake: native Codex delivery (#228, fixes #224)

• Codex replies are delivered with Codex's own codex queue --thread $CODEX_THREAD_ID --message ....
  codex-dynamic is no longer required for github-wake.
• --codex-home DIR selects the Codex account. Without it, $CODEX_HOME is used, and if
  that is unset, the one ~/.codex* directory that holds the session (Codex does not
  export CODEX_HOME to tool shells).
• An idle Codex session starts a new turn right away. A busy session gets the reply
  when its current turn ends.
• After an upgrade, a watcher still running an older version is replaced automatically.
  Watches armed through the removed codex-dynamic path now fail with a hint to re-arm,
  instead of retrying forever.
• Repository new-issue alerts (--repo) are tracked separately in #226.
