Use VSCode integrated terminal for proper TTY support

- Removed webview/xterm.js approach and child_process complexity
- Use VSCode's built-in terminal.createTerminal() with proper TTY
- Fixes interactive input commands (like nu's 'input') that need real TTY
- Much simpler and more reliable - no pseudo-terminal workarounds needed
- Terminal handles stdin/stdout/stderr natively with full interactivity

This fixes the "I/O error" when running interactive Nushell scripts.
