Code Refactoring

• drop cjs build, esm-only package (0cc2a4f)
• drop cjs shim, go full esm-only (d3e41df)

BREAKING CHANGES

• the package no longer ships a CommonJS entry.
  require() on Node >= 22.12 loads the ESM namespace ({ default })
  instead of the plugin factory; on Node 22.0-22.11 it fails with
  ERR_REQUIRE_ESM. Use import or stay on v2.x for require()-based
  configs.
• dist/index.cjs is now an interop shim for the ESM
  output; loading the package with require() needs Node >= 22.12
  (require(esm)). On Node 22.0-22.11 or with require-only tooling on
  older Node, use ESM or a bundler instead. The plugin API (default
  export factory) is unchanged.
