Using slashes in host input box generates error

I use mod_proxy to route requests to various ES machines based on hostname.

Example:
http://example.com/elasticsearch/node1/
vs.
http://192.168.0.10:9002/

This is actually two separate issues. Well, one issue and one enhancement.

The above commit includes a hack to fix twbs/bootstrap#5566

jq_changeHostButton() now strips trailing slashes to work around the naive concatenation that happens in poll(). It will now also reload the indices on host change.
