• Added: stack traces start at the throw site. The exception's own file and
  line travel as frame zero, since PHP's trace starts at the caller and never
  held the failing line. trace_limit counts that frame.
• Added: application frames carry a source snippet, the five lines on each
  side of the frame's line, so the dashboard shows the failing line in context.
  WordPress core, wp-config.php and the dependencies never carry one, and a
  line that names a secret and gives it a value is masked whole. The
  code_snippets option key (default on) switches it off.
• Changed: the bundled core is required as ^0.3.0.
