Revise lifecycle of a patch chapter

I think the sections currently ordered as

• What tools are required to submit a patch?
• How to prepare a patch?
  • Using a git mirror

would be better as

• Preparing a patch from the SVN repo
  • The information from What tools are required to submit a patch? should be incorporated as part of this section (not necessarily a sub-section, but could be).
• Preparing a patch from the GitHub mirror

This would be a more logical order and also make the GitHub mirror workflow easier to find.
