Ridu 0.16.2

This patch lets @riducms/sveltekit run in SvelteKit 3 applications as well as SvelteKit 2.

Fixes

• @riducms/sveltekit failed to type-check in a SvelteKit 3 application. It imported the Handle
  type from @sveltejs/kit, which SvelteKit 3 exports only from @sveltejs/kit/hooks, and read
  browser from $app/environment, which SvelteKit 3 deprecates and ships no types for.
  createRiduHandle now returns a RiduHandle that both versions accept as a Handle, so it
  still composes with sequence. The package reads the browser flag from esm-env, which
  $app/environment re-exports in SvelteKit 2. The peer range is now
  ^2.20.0 || ^3.0.0.
• The SvelteKit guide shows how a SvelteKit 3 application declares the Ridu URL in src/env.ts,
  and where sequence and Handle come from.

Upgrade

Update the Go module and all Ridu npm packages to 0.16.2 together and rebuild with the matching CLI.
This patch changes no Go APIs, manifest or protocol versions, plugin contracts, migration
planners, or database layouts. @riducms/sveltekit adds esm-env as a dependency. SvelteKit 2
applications need no changes.
