• A synchronous GC ran while requests were in flight. main.ts passed
  every dependency to createGatewayApp except the runtime sampler, so the
  app's bindInFlight call no-op'd on an optional dependency and the sampler
  kept its () => 0 placeholder. cartethyia_process_in_flight reported zero
  forever while proxy_in_flight — the registry the placeholder was meant to
  read — reported real flights.

  checkIdleReclaim gates on inFlightSource() > 0, so a permanently-zero
  source told the gateway nothing was in flight and let it run a synchronous
  Bun.gc(true) under load. A synchronous collection stalls concurrent
  requests: latency pinned to the request deadline and clients cancelled en
  masse with 499.

  Both classes behaved exactly as written — only the wire between them was
  missing — so it is pinned by a composition test that reads main.ts and
  requires the field.
