Add Terraform IaC section (11 tools, three entry points), HuggingFace
local fallback section (priority chain, model details), new env vars
table entries (TF_*, HF_*), and install variant examples.

Move v3.0.0 release notes into collapsible changelog. Update tool count
to 245+.

Also fix HTTP transport missing TerraformRunner and HuggingFace client
initialization (was only wired in stdio transport).
