Install the Lens agent command for Claude Code. Lens had a full definition and manifest entry but no Claude Code command wrapper, so it never installed for Claude Code while all sibling agents did. Adds the missing lens.md command.
Guardrail:verify-manifest.js now asserts every _sam/agents/ manifest row has a matching Claude command file, so a defined-but-uninstalled agent fails npm test instead of silently vanishing.
