BREAKING CHANGES

• lint: both rules are on by default, and the formatter now
  edits the <h4> SAS Macros </h4> section of a header. sasjs lint
  reports undeclared and unused macros that it previously ignored, and
  sasjs lint fix (or a format-on-save) adds the macros a file uses and
  removes the ones it does not - a removal drops a dependency the compiler
  consumes, so a macro referenced only indirectly, through a dynamic
  %&name call or an included program, is no longer listed. The default
  header template also gains a blank line before the SAS Macros section
  and after its list. To keep the previous lint output, set
  "noUndeclaredMacros": false and "noUnusedMacros": false in
  .sasjslint.
