Phase 1 MVP

Functional end-to-end MVP:

• Intake form — paste a script or upload a .docx
• Server-side validation (zod) and .docx text extraction (mammoth)
• Persisted to Supabase (submissions table, RLS enabled)
• Slack notification fires on every submission
• Accessible UI (shadcn/ui on Radix primitives), targeting WCAG 2.2 AA

Verified live: a real form submission produced a row in Supabase
(slack_notified: true) and a Slack message in the configured channel.
