Installation migration

If automation should install every listed Skill, add --all to skilld add.
Agents, pipes, CI, and --plain require this choice when several Skills are listed.
Hosted delivery failures no longer switch to direct GitHub installation automatically.
If you choose installation without an Artifact attestation, add --direct.
Direct installation records unverified. Behavior approval still applies.
