upgrades: Make documentation consistent

Ensure that all commands in the upgrades user-facing documentation is consistent in the following areas:

• We use double dashes for all migrator arguments, not single dashes.
• We use a $ (or don't?, depending on consistency with other docs) as a prefix to differentiate commands from terminal output.
• We make all commands copyable but fail-fast if they need user input.
