Switch streaming to Opus via /audio/:/transcode/universal/start

Replace MP3 transcode URL with Opus/OGG endpoint matching Plexamp's
exact streaming params (directPlay=0, musicBitrate, X-Plex-Chunked=1,
X-Plex-Client-Profile-Extra as URL param, X-Plex-Session-Identifier).
Enlarge stream_url and DownloadCtx.url from 512 to 2048 to fit the
~650-char Opus URL. Fix temp file extension: mp3 → opus.
