• Centralized INSERT, UPDATE, and DELETE executor failure translation, safe or opaque Tool-response message selection, and UPDATE/DELETE batch-failure text in one private module with explicit operation-specific policies.
• Preserved exact messages, validation Error identity, original causes, constraint matching priority, direct-cause inspection, DELETE safe-prefix exposure and cascade guidance, and non-Error handling.
• Preserved public APIs, response shapes, logging, timing, INSERT cause metadata, retries, partial-success behavior, transaction participation, and session ownership.
• Added executor and Tool characterization tests and focused policy coverage for validation allowlists, constraint variants and precedence, wrapped errors, opaque outcomes, and batch failures.

Validation

• pnpm release:validate passed with 249 test files passed, 2 skipped, 3,176 tests passed, and 23 skipped.
• Skips are intentional opt-in coverage for the Neo4j integration service (13 tests), PostgreSQL connection tests (4 tests), and web-search performance tests (6 tests); enable them with a running Neo4j service and RUN_INTEGRATION_TESTS=true, POSTGRES_CONNECTION_STRING, or RUN_WEB_PERFORMANCE_TESTS=true plus network access, respectively.
