Replace automatic inline block generation with explicit tts:display='inlineBlock'.

The discussion of #146 led to a conclusion at the London F2F to require the use of an explicit value on tts:display to obtain inline block formatting, as opposed to the automatic generation of an inline block based on specific circumstances.

Consequently, this issue will effectively remove this automatic behavior and define the explicit behavior for generating an inline block.
