Reorder signup fields

Context

Currently the sign up fields are ordered with Name after Password. Generally in sign up flows the Password field is the final one. This has caused some users to enter their password into the Name field as typically the only situation where a field is below a password field is when users are asked to confirm their passwords.

Tasks

• Reorder the fields so that Name is at the top, above Email
