• The MCP guide describes project-decision permissions and the accepted facts and numbered tasks that
  confirming a proposal can create, alongside the owner-only review of meeting findings.
• Key-document suggestions report state: "untracked" because no source fingerprint was saved with
  the suggestion. They no longer incorrectly report unchanged text; reread the document before deciding.
• Project proposal review includes document paragraphs, using the shared Core passage helper around
  the finding's words. projects actions list, its MCP counterpart and key-document review now show this
  context; --brief or zero before/after counts still omit it. The guides also explain oversized evidence
  and accepting unchanged findings from a partly stale batch.
