Added

  • Track changing decisions with facts add --supersedes fact:ID and facts reverse.
    Replacements supersede the earlier decision on acceptance; reversals keep its history.
    Both require the same imported document and account, use shared Core fact transitions,
    and respect offline, read-only and dry-run controls. facts add --occurred-time records
    the evidence’s event time.
