Add an extra column to the data set summary to indicate it was published via Monitor

As discussed in this morning’s meeting, this function is to add another column to the ‘Data set summary’ screen. The new column should be:

Heading ‘Created in’
Options = Monitor, MERIT
Rule to apply
Any time the ‘New data set summary’ button is pressed (on this screen), the row would display ‘MERIT’.
If the information originated in the Monitor app, the row would display ‘Monitor’ (even if modified further in MERIT). This would show what would be captured in the BDR.
