A positive maxCostPerRun now covers /v1/messages

Behaviour change: a positive cap only covered /v1/chat/completions, so a client
on /v1/messages, such as Claude Code, could spend past it without limit. If you set
a cap and call that route, you now get a 429 once the budget can't cover the next
paid call.

• Shared budget: the route draws on the same session budget as chat, on the wallet
  and API-key rails. Send x-session-id to keep both on one budget. Without it, the
  session comes from the first user message, and clients that format that message
  differently on each route get separate budgets.
• No downgrade: the route forwards the model you asked for, so both modes refuse
  rather than swap in a cheaper one. Strict mode answers cost_cap_exceeded. Graceful
  mode answers budget_exhausted, with X-ClawRouter-Budget-Mode: blocked.
• Free models still answer when the budget is gone.

Image, video, music, speech and partner routes are still outside a positive cap.

Docs

The routing docs, README and skill still listed Llama as free and left out Bolt and
Super 120B, and the README and skill still mentioned Sora 2 after v0.12.308. These now
match the live catalog.
