Fix gh CLI detection bug and improve no-access feedback
- Fixed a bug where users without GitHub CLI installed would incorrectly see the "no access" screen instead of the setup instructions
- Added docs link and direct-clone fallback to the GitHub CLI install screen
- Added Requirements section to README (Git + gh CLI prerequisites)
