界面中英双语 + 修复「取不到模型名」

• 界面所有文案（P 芯片、弹层、设置页、按钮提示、读屏 aria-label）改为中英两套，跟随 DSH 的语言设置自动切换。
• 修复：模型没有推理等级时（模型选择按钮的 title 就是纯模型名），插件找不到该按钮，P1 行会显示占位「当前会话模型」而不是真实模型名。现在中英两种语言下都能正确取到。
• README 补充双语说明。

UI strings now ship in both Chinese and English and follow the DSH language setting; also fixed the session-model name lookup when the model has no reasoning-effort label.
