AI usage notifications

• Get notified when AI usage crosses a threshold. Three configurable alert points (80/90/100% by default), each of which can be turned off individually.
• Optional notification when the usage window resets, gated on how much of the previous window you actually used — so you only hear about it if you were near the limit.
• Alerts fire once per window and never repeat while you sit above the line.

Fixes

• The AI usage indicator now shows an error symbol instead of an empty ring when usage can't be read, so a failed request no longer looks like zero usage.
• Fixed silent Keychain write failures that could leave the app reporting "authenticated" with no credential stored.
• Changing the polling interval now applies immediately instead of waiting out the previous interval.
• Fixed a latent crash in the system HUD event path.
