This brings v2 codebase in line with v3 with regards to manual page generation.
I removed the inline manual page substitutions (paths or compiled CNID backends) like we did for v3, which we concluded was not worth the effort and complexity.
Adds a 'make html' step to the Ubuntu integration tests job, to test the manual page generation in CI.
