Preserve DOCX title and introduction paragraphs before the first section heading. Support explicit same-document repair with reparse=true and an existing document_id, while preserving ordinary duplicate-content rejection and active-job protection.

Reviewed in PR #493. Complete PR CI passed (lint, typecheck, worker/API tests, CodeQL and secret scanning). Coordinate Hipo SDK 2.8.1 and same-Source repair admission. Original 25 MB DOCX live acceptance remains pending production rollout.
