Critical fix: the llm/stream waterfall listener must return an AsyncIterable synchronously. The previous async listener returned a Promise, and the upstream dsh-session-checkpoint-policy's yield* next() threw TypeError: yield* (intermediate value) is not async iterable, failing the whole turn before any chunk arrived. Fixed + regression test (26/26).
