Self-hosted counter API

Migrated from counterapi.dev to our own Cloudflare Worker + D1. The free tier was hitting 503s under our volume (~2K writes/day). New infra has 100K writes/day headroom.

Server-side analytics (no client changes): geographic distribution, daily unique users, activity curves — all via CF headers logged on each increment.

Also included

• PR #76 (@ynaamane): Opus 4.7 model detection (1M context)
• PR #75 (@bslinger): Claude PID handoff for detached guard (prevents SIGTERMing wrong process)
• Fix #77: guard crash when --session receives a transcript path
• python3 -m pip fallback in hook upgrade chain (schema v5)
