기존 Orchestrator SKILL.md가 "MCP 도구가 사용 불가능한 환경에서는 추적 없이 기존 방식대로 실행" 문구로 soft escape hatch를 허용하여 실제 세션에서 orchestration_start 호출이 누락되는 문제 발생. 3중 방어로 강화:

  1. SKILL.md Phase 0 (오케스트레이션 등록) 최우선 승격 + soft escape hatch 제거
  2. hooks/audit-orchestration.sh 신설 — 세션 >=180초 + orch 업데이트 0건 → orchestration_missing 이벤트 자동 기록
  3. tests/test-scripts.sh에 상시 assertion 8건 추가

검증: 로컬 + CI=true 양쪽 410/410 PASS
