Pools several OpenCode Go subscriptions (one API key each) behind the single provider route the agent already uses.

1. The route's apiKeyEnv points at a pool-owned credential reference; the plugin writes the healthiest key into it before each request.
2. Health comes from the gateway usage endpoint (5-hour / weekly / monthly windows): a spent key never reaches the wire, and a stale reading refreshes in the background.
3. On HTTP 429 + GoUsageLimitError it cools that key for the window reset and re-dispatches the same frozen request against the next key - only while the request has produced no token yet.
4. Misconfiguration fails loud at activation and names the offending field.
