Lost-track restart. A run whose worker terminal is gone, or that stays idle after a check-in, before any PR is
aborted and re-dispatched from scratch (bounded, worker.lost-tracking/worker.restarted) (#147). It never
destroys work: it needs the evidence on two consecutive passes, escalates stuck and keeps the worktree when there
are unpushed commits or uncommitted edits (the check fails closed), removes a clean worktree without --force,
captures the worker screen into the run first, waits while the tracker is cooling down, and does nothing
destructive until its tracker writes succeed. A human Retry resets the restart budget (#151).
A worker that declares itself blocked (BLOCKED: worktree comment) is escalated to a person — also when it
exited afterwards — instead of being nudged, handed off or restarted (#150, #151).
Never type into a bare shell. A nudge into a terminal that is only a shell prompt relaunches the agent (the old
terminal is closed first; an idle agent from a CLI that does not register with Orca is never mistaken for a shell);
no-PR worker events keep the issue running in the UI (#144, #151).
Close-or-reopen decisions on an abandoned delivery stick and release the issue's lease (#145, #151).
UI. Worker slots are counted the way the tick counts them (#148); the projection rebuilds itself when the
reducer changes (#149).
