scmBayesPost v0.4.5

What changed

build_iv_formula() is now a formally exported package function.
Previously it had to be sourced as a standalone script. It is now
available directly after library(scmBayesPost).

Formula attributes (treatment, instruments, controls) are stored
at creation time for reliable downstream parsing by
instrument_validity_report() and test_instrument_strength().
