A patch answering a report from teams sharing one repository: decide no
longer reports "a question it closes" as missing, and the help, cheat
sheet, README and gy-loop skill now say that next/handover take --scope,
that list --since <seq> excludes that sequence, and that each team names
its writer apart. The storage format, gy.toml and the commands are
unchanged.
