Lean workers: spawned dsh runs can start at ~4k context tokens instead of ~13-15k.

• DSH_WORKER_PATCH — when set in the omp session environment, both bridge runners (runDsh sync, startDsh async) prepend --patch <path> to the dsh argv. Point it at a lean overlay (13 plugins off + worker persona) and a delegated run carries tools it uses instead of the full global composition (measured on DSH 0.2.0-rc.2: 46 tools/11.2k tokens of descriptions down to 17/3.4k, 45 skills to 0).
• dsh-task env forwarding — the extension previously swallowed the session env, so no environment flag could ever reach the spawned dsh. Both startDsh call sites now forward { ...process.env }; the bridge still applies its own allowlist on top.
• Tests — 3 new bridge cases (async appends --patch, async omits it unset, sync appends), bridge suite 167 pass; strict tsc and Biome green.
