Add documentation XML comments on IDistributedCache and associated classes/types.

At the very least a high level summary for IDistributedCache + descriptions for the behaviour of the main operations (Get/Set/Refresh). E.g. I'm assuming Refresh() is mainly to 'touch' entries with a sliding expiry, etc.
