Added

• Opt-in CapturePolicy::ProviderTracked reuses a fully verified provider observation when SQLite proves it is unchanged, or verifies only a proven append suffix. SQL changes from other connections invalidate reuse. Every open still verifies the complete store; raw file edits bypassing SQLite are outside the warm tracking guarantee.
• Scoped recorded-facade reads, including one-call read_histories that preserves requested order and duplicate subjects. Existing constructors retain full verification.
• Executor::execute_versioned and batch_versioned let callers select an exact definition version and receive input-guarded refusals before subject existence or revision checks. Existing execution methods keep selecting the definition from the loaded subject.

Fixed

• Preserve a declared service refusal when a command supplies fulfillment keys for its success outcome, including guards that inspect the loaded subject.
• Return a declared creation refusal before an existing-subject conflict. Exact retries still recover their original result before evaluating a new decision.
