Restyled exclusion picker, colored chips for plugin items

Changes

• Keep the search bar and dialogs from blocking each other

  A dialog no longer opens behind the always-on-top bar: opening one hides the bar,
  summoning the bar while a dialog blocks it brings that dialog forward instead, and
  dialogs of Settings block only Settings, so the bar stays usable meanwhile.

• Restyle the exclusion picker and its custom rule editor

  • The picker and the rule editor use the InfoDialog look of the other dialogs,
    with an eye-off badge, a footer bar and a filled Exclude button
  • Rows show a file or folder icon, the name above its location and a pill with
    the number of entries; the result itself reads "Only this entry"

• Let plugin items show colored chips, such as a ticket's status

  • PluginItem.chips takes Chip(text, tone); the tones neutral, info, success,
    warning and danger map to theme colors, so chips read well in light and dark
  • Up to three chips come before the location; the location gives way first,
    screen readers hear them, and the item cache keeps them
  • Chips are shown, never matched; docs/plugins.md explains them

• Render README screenshots off screen so input elsewhere cannot reach them
